Job Description:
A company is looking for a HEDIS Outreach and Engagement
Therapist.
Key Responsibilities
Engage recently discharged patients through proactive outreach and
conduct telehealth sessions for follow-up behavioral health
visits
Document screenings and provide evidence-based psychotherapy,
developing individualized treatment plans aligned with discharge
goals
Collaborate with care teams to ensure timely discharge
notifications and maintain accurate documentation in EHR
systems
Required Qualifications
Master's Degree in Social Work, Counseling, Psychology, or a
related field from an accredited institution
Active and unrestricted LMSW, LCSW, LCPC, LPC, LMFT, or equivalent
clinical license required, or ability to obtain within three
months
Minimum 2 years of experience in behavioral health clinical
settings
Crisis intervention training and suicide risk assessment competency
required
Prior experience with HEDIS metrics or value-based care initiatives
is highly desirable
