Job Description:
Description Summary: The Medical Coordinator position is
responsible for arranging and coordinating appropriate health
services, including urgent and preventative health services, for
children in care. This includes maintaining complete and
confidential medical records for children, ensuring appropriate
communication with health needs to case managers, clinicians, and
other team members. The position schedules appointments,
coordinating transportation, and may assist as needed to health
care appointments and ensuring qualified translation/interpretation
services are in place. He/she also serves as a liaison for
community health care providers. He/she operates under the
supervision of the Lead Medical Coordinator (LMC) but may fill in
when the LMC is not available. The Senior Medical Assistant handles
incoming calls timely and efficiently, takes accurate and
appropriate messages, and assists and greets children/youth in a
friendly and courteous manner. Obtain, compile, and maintain
medical files on each admission. Ensure proper daily documentation
in the ORR database, UAC Portal, and maintain a hard copy of the
required documents and medical records. Assists other departments
with various duties as needed. Roles and Responsibilities: Obtain,

comply with required medical and dental appointments and timelines.
Respond and comply to individual emergency medical needs
immediately. Maintain and review for accurate and complete list of
medical supplies, prescribe medications, and over-the-counter OTC
medications according to the standing orders. Maintains, updates
and creates all monthly medical sheets. Prepare youth for
examinations and record the medical intake, take vital signs,
height and weight. Initiate routine lab tests. Participate and
advocate during the medical procedures performed by medical
providers and other health services. Submits TARS for Shelter
clients. Assist the medical providers’ staff with examination
procedures when needed, such as patient screening and triage.
Dispense medication according to self-administration of medication
- SAOM schedule, policies and procedures. Collect, maintain, and
submit weekly, monthly, quarterly, and annual reports to
supervisor. Schedule and comply with required medical appointments
and timelines. Provide transportation to clients on medical
appointments with assistance from floor staff as needed. Respond
and comply to individual emergency medical needs immediately.

Order medical supplies, and medication when necessary.
Facilitate/conduct universal precautions, TB 101, allergies/other
related training to employees. Provide fail-safe plan with
medication count, label accuracy including over-the-counter drugs.
Educate and counsel youth concerning their disease, treatment, and
prevention of disease. Schedule and develop and expand medical and
dental and other related services with local providers. Able to
react to change productively and handle other essential tasks as
assigned. Maintain Health Information Privacy HIPAA. Submit, review
and follow up all the Significant Incidents Report regarding
Medical Issues. Participating in Quality Improvement as required
Keeps all office equipment operating by following operating
instructions, troubleshooting breakdowns, maintaining supplies,
performing preventive maintenance, and submitting service requests
for repairs. Function as Lead in the absence of the Lead Medical
Coordinator Other duties as directed Qualifications Qualifications
Licensed practical or vocational nurse (LPN/LVN). Or a bachelor's
degree in a related field (example, nursing). Minimum 1 year of
full-time experience in a public health care or healthcare setting
with demonstrated experience in care coordination, health
administration, and/or direct client care. Excellent
organizational, verbal, written, and interpersonal communication
skills in English and Spanish required Ability to easily
collaborate with other staff and service providers Proven ability
to contribute both independently and as a key team member
Demonstrated success working and communicating effectively in a
multi-cultural environment Self-starter with excellent
problem-solving skills, combined with the ability to prioritize
duties independently Must have a valid California driver’s license
and good driving record. Possess or be able to obtain CPR and First
Aid certificates. Have and maintain Professional Assault Crisis
Training (ProACT) and/or Crisis Prevention Intervention Ability to
successfully pass a criminal background check. Must be at least 21
years of age. Physical Demands: While performing the duties of this
on-site job, the employee is regularly required to sit; use hands
to finger, handle, or feel; and talk or hear. The employee is
frequently required to walk. The employee is occasionally required
to stand and reach with hands and arms. The employee must
occasionally lift and/or move up to 25 pounds. Specific vision
abilities required by this job include close vision.
