Job Description:
A company is looking for a Transition Specialist who will
support members transitioning from inpatient hospitalizations.
Key Responsibilities
Support care coordination teams and providers in developing
effective transition plans for community members
Assist with member transitions based on enrollment or care services
identified
Track and maintain transition metrics to identify trends and
process improvements
Required Qualifications
Bachelor's degree required
2 - 4 years of related experience preferred
Experience in service coordination, case management, or discharge
planning
Knowledge of community advocacy and transition of care
processes
Equivalent experience may substitute for formal education
requirements
